Transitional Housing Placement-Plus Foster Care (THP+FC) refers to a program that provides housing and supportive services to former foster youth, classified as “Non-Minor Dependents” (NMDs), who are between the ages of 18 and 21, allowing them to continue receiving foster care benefits while transitioning into independent living; essentially, it’s an extended foster care option with additional housing support and life skills development services

Referral: Must be referred by a Child Welfare Worker

To assist and support NMDs in their transition from foster care to full independent living.
To build a collaborative relationship with each NMD and create lasting connections with supportive adults as they transition to full independence.
To proactively help NMDs achieve their independent living goals as outlined in their Mutual Agreement.
To provide real-life experiences that promote independence. To allow NMDs to learn from their mistakes and make necessary adjustments for success.
To offer a supportive safety net that nurtures a strengths-based approach, guiding NMDs toward successful independence from foster care.

Our services are tailored to each participant’s unique goals, identified collaboratively with their County Social Worker, and documented in the participant’s Mutual Agreement and Needs and Service Plan.

Support Services: Based on the Transitional Independent Living Plan (TILP)
