Transitional Housing Program (THP)
Instructions
The THP application, referral recommendation, and symptom checklist are all required.
The youth applicant may complete the application, but the referral recommendation and symptom checklist must be filled out by a worker associated with the youth.
Completed documents can be submitted in one of the following ways:

Additional requirements
IMPORTANT NOTE: TB (Tuberculosis) tests are a requirement prior to a youth move-in.
All housing is on a first come first served basis.
